Summary of Forms
The following three forms are found on the Tenant Forms page. They consist of:
- Tenant Administrator Designation Form
- Access Card Request Form
- Emergency Contact List
Each Tenant should ensure that a completed set of these forms is on-file in the Management Office to enable personnel to:
- Identify authorized Tenant Administrators
- Accommodate authorized after-hours access requests
- Authorize lock & key work and/or reimbursable work projects
- Contact you in the event of an after-hours emergency
- Comply with Fire Department Life Safety Training Requirements
